About Our Opportunity:

In our community based residential facility, the Home Support Worker is responsible to and works under the direction of the Senior Support Worker in providing care to the residents, which includes meeting their physical, social, and psychological needs. The Home Support Worker will provide the daily delivery of client supports and domestic housekeeping to individuals living in our homes.

 Job Duties:

 Review client background information, interview clients to obtain case history and prepare intake reports; Assess clients' relevant skill strengths and needs; Assist clients to sort out options and develop plans of action while providing necessary support and assistance; Refer clients to other social services or assist clients in locating and utilizing community resources including legal, medical and financial assistance, housing, employment, transportation, day care and other services; Counsel clients living in group homes and halfway houses, supervise their activities and assist in pre-release and release planning; Participate in the selection and admission of clients to appropriate programs; Meet with clients to assess their progress, give support and discuss any difficulties or problems; Assist in evaluating the effectiveness of treatment programs by tracking clients' behavioural changes and responses to interventions; Provide crisis intervention and emergency shelter services; Maintain contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients' overall progress; Maintain program statistics for purposes of evaluation and research; Supervise social service support workers and volunteers.

 Grade K- 12; Certificate or job reference as Mental Health Support Worker or an acceptable work experience in a related field of work;  8 core competency courses as mandated by the Department of Community Services (can be provided by employer). Previous work experience supporting adults with mental health/intellectual disabilities. Proven ability to work effectively both independently and collaboratively in a fast-paced environment that can at times be volatile.

 The successful candidate must have excellent written and verbal communication and possess sound judgment and high-level diplomacy skills. The successful candidate shall demonstrate an ability to write reports and maintain accurate records/files; prepare healthy meals; maintain a highly sanitized facility and engage in standard yard maintenance activities. He/she must be a team player/leader demonstrating a high degree of initiative, and a positive can-do attitude. Commitment to social inclusion and inclusive community living.

 Work Conditions and Physical Capabilities: work under pressure, repetitive tasks, physically demanding, attention to detail.

Salary: Starting at S18.00 per hour, depending on qualifications.
