Health Care Aide
Tiverton Park Manor is a Retirement Community in Tiverton, ON providing assisted living to our 50 residents in a friendly, modern, homelike atmosphere. You will join an excellent team of RNs, RPNs, PSWs and support staff.

Currently seeking to fill a full-time position, for a minimum 30 hours per week, with Day and Evening shifts, as well as an option for overnight companion shifts in our residential hospice suite.

Primary job responsibilities:
- Assists in individual health care issues by observing and reporting changes in the physical and emotional condition of the resident. Provides assistance according to the service plan.
- Participates in the overall and specific programs developed by the facility.
- Escorts residents to specific programs and activities of daily living as required.
- Performs personal care
- Performs selected controlled acts of an RN only after completion of the procedure necessary to perform them as an unregulated care provider such as glucometer checks.
- Gives oral, and topical meds after completion of training and examinations.
- Records services performed for each person as instructed.
- Maintains a safe environment for residents and staff by following safety policies and procedures and reporting hazardous situations in the appropriate manner. Documents incidents verbally and in writing.
- Participates in service conferences to plan, develop, and assess individual service plans and review medications.
Minimum qualifications:
- PSW Certificate or equivalent experience as Health Care Aide is required.
